IRS Opium Order Form

From the 1930s and 1940s
This official Internal Revenue Order Form was used by doctors, pharmacists and hospitals for ordering opium and cocaine derivatives by prescription, which were declared taxable controlled substances in 1914. Printed on Internal Revenue security paper, it features an ornate border with a picture of an opium poppy. This rarely seen 8 ¼ x 10 ¾ inch conversation piece is perfect for framing!
